There are several ways you can apply for these programs.

Depending on the program you are applying for, you may, or may not be required to have a face-to-face interview at our office. If you are required to have an interview, you will be notified by mail.

Interviews for Cash and SNAP Benefits are conducted by phone or at our office Monday through Fridays. Appointments can be made over the phone, in person, or through the mail.

IMPORTANT: If you have a case open in a different county, that county must transfer your case to Geauga prior to your appointment.


Apply in-person

You can receive and complete an application in person at our office at 12480 Ravenwood Dr. Chardon (next door to Geauga University Hospital).

You can call our office to request an application be sent to you in the mail. Once you receive the application, you will need to complete it and return it using the business reply envelope that will come with it. You will receive a follow-up letter with further instructions after we receive your completed application.

Once you have downloaded and printed the application, complete it and mail to: Geauga County Job and Family Services, PO Box 309 Chardon, Ohio 44024. You will receive a follow-up letter with further instructions after we receive your completed application.

You can also check the status of your application and/or report changes pertaining to your existing case.
