Lump Sum Notification Process
- In accordance with ORC section 3121.07 you are required to notify the Child Support Agency of any upcoming lump sum payments (bonuses, commissions, or severance pay) to this employee/obligor, in the amount of $150.00 or more, no later than 45 days before the lump sum payment is to be made.
- You can email this notification to: Geauga-CSEA-Lumpsum@jfs.ohio.gov.
- Please include Obligor name, case number or other personal identifier, and return contact information.
The National Medical Support Notice
This document serves as legal notice that the employee identified on the Notice is obligated by a court or administrative child support order to provide health care coverage for the child(ren) identified on the Notice.
